Office Policy – We require 24 hour notice on cancelled appointments. Missed appointments without a prior call or email to the office will be charged in full.
To schedule an appointment:
- Book your appointment. No walk-ins will be seen due to necessary pre-screening procedures.
- Complete the Ontario COVID-19 self-assessment questionnaire at the time of booking. You will be screened again prior to your visit.
- Patients must wear a mask at all times while in the office.
- Clean hands immediately upon entering and immediately following your appointment. Hand sanitizers are available throughout the office.
- Arrive on-time for your visit. This will help limit the number of people in the waiting room and ensure we are able to finish appointments on time in order to accommodate cleaning practices between patients.
Billing & Insurance – Direct billing is available for the following companies: Benefit Plan Administrators, Blue Cross, Canadian Construction Workers Union, Chambers of Commerce, CINUP, ClaimSecure, Cowan, Desjardins, Equitable Life, First Canadian, GMS, GroupHEALTH, GroupSource, Group Medical Services, Industrial Alliance, Johnson, Johnston Group, LiUNA Local 183/506, Manion, Manulife, Maximum Benefit, MDM, Nexgen RX, Sun Life, Telus AdjudiCare and the Co-Operators. Please note, some plans may not permit direct billing.
